For me too, it seems to solve this problem when modem drivers are removed... 
for some reason pulseaudio just doesnt want deal modem and my alc600-vd. here 
is my list:

Strange thing is that alsa detects modem as audio, i think it should be
disabled for alsa to think modems are the same sound cards as HDA ATI
SB, so pulseaudio get confused between these 2 things. Maybe i am
wrong...
