Ok I've now experienced this in skype as well, except input stopped
working. This crashed my system. Perhaps this has something to do with
how applications register with pulse audio, So I'll change this bug as
affecting pulse audio

  Firefox loses it's connection to PA whenever I leave my computer idle or
  suspend with firefox still open. It takes awhile but I'm sure if you let
  firefox just sit open for a few hours you could recreate the bug. It
  could be flash or firefox
