You didn't attach any keymap -i output. Did you use
/lib/udev/findkeyboards and had keymap -i listen on the "AT keyboard"
one? Do you have any Fn keys which do not work, but do generate output
in keymap -i?
I checked your udev-db.txt, and I don't see other input devices which
might catch these keys. So this is a kernel bug at least, for not
exposing those three keys as input events (and not even ACPI events).
