I think this is a design feature to stop unwanted repeat playing of the sound.
I have done some tests and it appears to be that Thunderbird will not play the sound again after having once played it until you have actually done something in T/b. I found that opening an email (any email) or switching folder views was sufficient to get T/b to play the sound when the next email arrived.
