>Could be bug #460710, do you get the issue using an another theme? I've experienced the problem when using Human, Human-Clearlooks, Clearlooks, and LegacyHuman. It's hit-or-miss with all but LegacyHuman, which seems to tickle the bug 100% of the time. Selecting List View in the attachment bar has been a reliable workaround.
