Same here with a 00:1f.5 Multimedia audio controller: Intel Corporation 82801EB/ER (ICH5/ICH5R) AC'97 Audio Controller (rev 02)
Might it have something to do with the analog-output* configuration files in /usr/share/pulseaudio/alsa-mixer/paths ? After changing all the switch entries to "switch = on" the volume is no longer muted, although it's still set to zero.
