1. I don't know because it's persistent across sessions.  I would have
to fix it to reproduce it.  (i.e. it happened when I resized the window
and now, anytime I open Mines again, it's that way.)

EDIT: I logged in as another user and  discovered that this is caused by
changing the theme in Ubuntu.  If you customise a theme and choose the
"water vapor" theme  under the "window controls" tab, then the vertical
gridlines disappear.  I think this is an unintended consequence of the
theme and it may also have something to do with the way Mines renders
the tiles.
