I can attest to this. I installed a brand new Karmic from netinst CD and installed standard desktop. As soon as it booted I opened Synaptic and marked all upgrades and installed them. Right after I checked the status section and saw "Installed (manual)" contained all the entries from the original install. Trying to fix it I changed that by the menu entry "Automatically Installed", and now I'm seeing all of those (1120 out of 1276) labeled as "Installed (auto removable)".
"Installed (Manual)" I thought was supposed to be for packages installed from individual deb files, and "installed (auto removable)" for files that are not being used and no longer relate to an installed package (for example a library with nothing using it).
