OK, I did the following:
1. edited client.conf -> autospawn = no
2. pulseaudio -k and killall -9 pulseaudio
3. rm -rf ~/.pulse*
4. edited client.conf -> autospawn = yes
5. pulseaudio -vvvv > ~/refreshpulselog.log 2>&1

The same behaviour is occurring. i.e. when trying to open pavucontrol
(whilst running 5.), "Connection Refused".

Again, an example of a failure in the log is:
I: (alsa-lib)pcm_hw.c: open /dev/snd/pcmC0D3p failed
I: alsa-util.c: Error opening PCM device hdmi:0: No such file or directory

but /dev/snd/pcmC0D3p does not exist. However, this may be misleading.
