In #2 Steve asked if a window was presented - yes, but I had left the machine unattended for a while (probably about an hour), and the screen was completely frozen so I was unable to interact with the window - keyboard and mouse (both USB) failed to function. I am using a Belkin Flip 2 KVM switch and I'm not sure if the machine wasn't responding to newly detected USB devices (as these effectively disappear when the switch is turned over to another machine). I managed to SSH in to the machine and restarted then completed the upgrade using apt-get -f install once mounting / as RW. System now running fine (and super duper fast compared to 9.04 imho). I think this can stay closed, but I'm wondering if it might have something to do with libpam-usb? Or if you can let me know where to find some more information on the error then I'll use that.
