Thanks for your report. In order for us to determine the cause, I have a
few questions for you:

Can you tell us if the bug is reproducible, that is, can you make it
happen every time, or was it something that happened just once?

Also, what file-system was on your pen drive? I realise it would be FAT,
but which one, FAT12, FAT16, FAT32?

Do you know how it was mounted, that is, with vFAT, or with another
mount type?

When you reformatted your pen-drive, what format did you make it?

How big is the pen-drive?

How many files were on it at the time?

Can you provide us with the output to the following command by opening a 
terminal:
   dpkg -l dosfstools
