Uff, sounds like something messed up your system-wide installed icons.
notify-osd does not do any super weird tricks with icon-substitution.
Hm... what could you do to narrow this down a bit... ?
1.) Check if you see the correct icons when you do:
eog
/usr/share/notify-osd/icons//hicolor/scalable/status/notification-audio-volume-medium.svg
eog
/usr/share/notify-osd/icons/gnome/scalable/status/notification-audio-volume-medium.svg
2.) What icon-theme do you use?
3.) Does uninstalling awn fix the issue?
4.) Do you see the same with a fresh install of Ubuntu 9.10 (e.g. under
VirtualBox/KVM/VMware) and awn?
