Everything I've reported has been using metacity.  If I turn on desktop
effects (which switches to compiz, if I'm not mistaken), I don't get any
system bells at first.  If I enable the PC speaker (modprobe pcspkr and
xset b on), then I get the expected beeps with the expected repeat rate.

So point (d) is not a Pulse Audio issue, as I had claimed.  Should it be
reported again metacity, libcanberra, or both (or something else)?
Separately, there the issue of not having any bell in compiz, but my
feeling is that that would be suitably mitigated by addressing (b) [and
maybe (a) and (c)].
