Actually, this modem is currently out of my hands but please see my other bug report that I filed on another broadband dongle of mine. https://bugs.launchpad.net/ubuntu/+source/network-manager/+bug/493993 I thought that the issue was solved but it wasn't. I am attaching the two files that you mentioned above to that bug report.
