This is even worse in evince 2.28.1-0ubuntu1.2 using poppler 0.12.0-0ubuntu2.1. I cannot even scroll through the list of countries. Clicking on the element causes it to become obscured by a small rectangular frame which contains an even smaller grey rectangle (possibly a display area?) with < and > on either side.
Also the "Clear Form" button does nothing except remove the focus. Entering evince voteform_enterable.pdf at a command prompt gives the following error twice, in addition to displaying the document: ** (evince:2219): WARNING **: Unimplemented annotation: POPPLER_ANNOT_SQUARE, please post a bug report in Evince bugzilla (http://bugzilla.gnome.org) with a testcase.
