Although not completely intuitive, I think I have a fix for you: You need to right click on the the KMix icon in the tray and select the "Select Master Channel" option. Mine was set to PCM, which was binding the volume control the PCM, rather than master. I think that this was leftover from a dist-upgrade.
Why this can't be set when you select "show mixer window" is a design choice that I don't understand.
