Per #51, switching to the current wicd (1.6.1-3ubuntu1) 100% fixed this for me, although I don't expect that clears either the drivers, supplicant or NM, but it might be a place to draw comparisons from. Moving to wicd did mean I was forced to write my own ppp script for a 3G wireless card.
