Thank you for taking the time to report this bug and helping to make Ubuntu
better.
One question, do you have compiz activate?
Please, go to System -> Preferences -> appearance -> visual effects
and set it to: None (Provides a simple desktop environment without any effects)
If the title bar appears again the you have a problem with compiz and maybe
drivers.
If no, please write back to continue digging in the problem
