Sorry for the delayed response. We are on the image with id ami- 59b35f30. If it makes any difference, I think our problem may have been related to NFS in some way, as at the time we were sharing the volume over a "soft" NFS mount which may have led to the corruption and consequent bug. I don't have a ton of evidence to the idea, but we haven't seen any new inconsistencies using xfs_check since we switched over to using a hard NFS mount.
