I can reproduce this bug already without CUPS, by opening your PDF file,
printing it into a PDF file (evince re-renders the file instead of
simply passing it through, blowing it up from 97K to 1300K) and then
letting Ghostscript render it with the command line you have posted
here:

This takes more than 9 minutes on my laptop, whereas feeding your
original file directly into Ghostscript does the job in less than a
second.
This is a known problem of Cairo (the rendering engine used by evince)
and not a Ghostscript problem. See bug 419143.
