I spent some time this morning and I've traced the problem to rpc.statd
not starting.  I tried to run it manually in foreground with debugging
turned on and here is the result:

03/18/2010 14:12:53 rpc.statd[4739]: Flags: No-Daemon Log-STDERR
Cannot register service: RPC: Timed out
: unable to register (statd, 1, udp).
r...@cfevalpreprodvm:/etc/init.d#

The above led me to dig around for a cause and eventually I found that
the loopback interface was not configured.  Once the loopback interface
was up, everything began working correctly.

Problem solved.
