Ok, good. There are a few possibilities here; the most likely are that vga16fb is blacklisted somewhere but just not in the obvious place, or that udev for some reason can't find the module at boot-time when it goes to load it. Please reboot again with the same options, and this time run: sudo modprobe -b vga16fb and check /proc/fb afterwards.
