This usually means that you have a bad CD burn, a failing USB disk, or a
corrupt source image.

Please verify the md5 checksum of the ISO you've downloaded
(https://help.ubuntu.com/community/HowToMD5SUM).  Next, try burning the
ISO at a slower speed and running the "Check CD for defects" option by
pressing space when you see the keyboard icon, then selecting it from
the menu that appears.
