I followed the steps you suggested. The misbehaviour happened one more
time, and then went away. And then a few weeks later it came back. It
kept happening each time I woke the machine until I rebooted, and it is
not happening any more.

Facts that might possibly have some relevance:
1. Just before it started happening I had disabled and re-enabled Pulseaudio (a 
necessary step to get round an unfortunate design decision by the Skype for 
Linux developer).
2. I was noticing recently that sometimes Amarok2 would start hogging CPU and 
have to be killed. This wouldn't happen at the same time as the PulseAudio 
freakout.

I have now removed Amarok2 and reinstalled version 1.4. I will let you
know if the PulseAudio problem reoccurs.
