Did you blacklist dv1394 and video1394 as well? Does an /etc/modprobe.conf exist on Ubuntu? If yes, regenerate it by "update-modules" or delete the file. modprobe ignores /etc/modprobe.d/* if /etc/modprobe.conf exists.
Does the initrd contain ohci1394? Hard to tell how to check that, as I never create and use initrd myself. Maybe dmesg gives away whether ohci1394 was loaded early.
