This doesn't seem to be file specific as I just ran some tests on a blank document I just created and the problem still occurs. What I've found is that it seems to happen once a transition had been applied to a slide and then a custom animation is added. Sometimes the animation will be added, and others the program will crash. I've tested this with a "Title, Text" slide layout with the "Wedge" and "Venetian Blinds" custom animations and the "Wheel Clockwise, 1 spoke", "Wheel Clockwise, 2 spokes", and "Wipe Down" transitions. When no transition has been applied, I was able to apply nine out of nine custom animations on the slide, but with a transition, I was unable to add a custom animation to all nine points on the slide, so I suspect that the transitions may be the culprit. I hope this helps! ;-)
