This happens to me as well. This is NOT "hit the wrong button" thing.
This is on and off, i would say happen approximately every 2-3 times
(ie/ every 2-3 successful restart/shut down). I also have the same 3
problems with Thomas W. When the ShutDown/Restart bug happens, I also
not have sound and cannot mount any external/usb drive.  So, these 3
things seems to be related, and occur on the same time.  And I notice
this start to occur around/after May 31 for me though (I use Update
Manager almost everyday).

I'm on 10.4

I also read some people in the forum saying modifying /etc/default/grub
will fix this issue. However, I dont even have this file. is this
because grub Vs grub 2 ? anyways, that may not even be related.
