ubiquity's poor response to this error is bug 69470, and I'll have it fixed soon.
However, the fact that you got disk errors and EIO on a virtualised hard disk in VMware is sufficiently weird that I'm sending it over to the kernel team for analysis. Maybe it just means a disk problem on the host system?
