  A couple of questions:

1) When you say you set the drive up for RAID was that done with
software MD or was that using some inbuilt RAID controller on the
system?

2) When it fails, does it leave you with a shell? If so can you give the
output of cat /proc/partitions    cat /proc/mdstat  and hmm anything
else that sounds useful? (Maybe an ls of /dev/mapper and a pvdisplay,
vgdisplay and lvdisplay )

If you boot a rescue CD you might also want to see what's actually
created.
