Could you plesae obtain a verbose pulseaudio log, as outlined here:
https://wiki.ubuntu.com/PulseAudio/Log. Since you say that pulseaudio
doesn't always work, you may need to do this more than once to determine
when you have non-fucntional audio. Once you have a log from a
pulseaudio session where you have non-functional audio, please attach it
to the bug for further analysis.
