There appears to be clashing between different applications claiming the
device, there is at least rhythmbox and gvfs trying to load the device.
Is this an actual USB modem (or even integrated) or are you connecting a
smart phone over USB?
To begin with, please try to make sure that rhythmbox isn't running at
the time you plug the device, then attach the contents of
/var/log/syslog again, so we can try to see if the situation improves a
bit.
