This really sounds like a hardware problem. First on my suspect list is your hard drive. Have you tried using smartctl to get a S.M.A.R.T report? Or have you tried running diagnostics? During the part of the file system check where the completion percentage is going very slowly, do you hear any kind of regular clicking or grinding or whiring noises from the disk that sound unusual? That could be the sound of the disk trying and re-trying to read certain disk sectors, again and again and again, until it finally gives up and moves to the next disk sector (which is also bad). That would explain the very slow progress.
