Please provide the output of the following:
dpkg-query -W devmapper dmsetup libdevmapper1.02 udev volumeid
libvolume-id0 lvm2 lvm-common mdadm
Also could you try booting your system with init=/bin/bash and then
remove the symlink for this init script, followed by exec /sbin/init
See if your system boots fine then.
If it does, boot into single-user mode and then run the script with:
sh -x /etc/init.d/lvm2 >/dev/lvm.log 2>&1
and attach the output to this bug
