This normally means udevd isn't running properly.  Recently this seems
usually to mean that the udevd from the initramfs was not killed
before switching to the real root.  The effect is that the init.d
script sees `a' udevd running and doesn't start one, but the one
that's running is in an initramfs dreamworld.
