To summarize:

1. start jackd at 48kHz
2. run jack-dssi-host fluidsynth-dssi.so
3. load a sound-font. I used /usr/share/sounds/sf2/FluidR3_GM.sf2, but had the 
same problem with all other sound fonts that I tried.
4. press 'Send Test Note'

I hear loud white noise, with the proper note just audible.
