I've found that creating a .gtkrc-2.0 file in my home directory with the
contents:

include "/usr/share/themes/Ambiance/gtk-2.0/gtkrc"
include "/home/sean/.gtkrc-2.0.mine"
gtk-icon-theme-name = "ubuntu-mono-dark"
gtk-theme-name = "ubuntu-mono-dark"

mostly fixes the problem. It seems that gtk isn't picking up the
settings from gconf.
