Again, please attach /etc/pam.d/common-auth from this system.

In any case, Ubuntu 9.04 has reached end of life and no longer is covered by
security support.  Please upgrade to Ubuntu 9.10 or later and let us know if
the problem is still reproducible with currently-supported releases of
Ubuntu.
