Confirmed, exactly the same problem with Toshiba Satellite Pro P100 with
same wireless card (rev 02).

I also would much like my interface to work on boot up without having to
do the ifdown/ifup. I've also noticed that my boot process seems to hang
for about 20 - 30 seconds at configuring network interfaces, so maybe
that is related.

My /etc/network/interfaces:

auto lo
iface lo inet loopback
address 127.0.0.1
netmask 255.0.0.0

auto eth0
iface eth0 inet dhcp

auto eth1
iface eth1 inet static
address 10.0.0.13
netmask 255.255.255.0
gateway 10.0.0.2
wireless-key s:xxxxxxxxxxxxx (removed)
wireless-essid ginger.wifi

auto eth2
iface eth2 inet dhcp

auto ath0
iface ath0 inet dhcp

auto wlan0
iface wlan0 inet dhcp
