Mind posting a screenshot or photo of the screen showing the missing
window bars?
Offhand, it sounds like a window manager failure in showing the window
decorations, but a screen image would tell for sure.
Look in your .xsession-errors file to see if it shows the window manager
crashing.
What change did you make in the X failsafe mode, that enables you to
reboot into Ubuntu normally?
When you log in, are you choosing the Classic Desktop or the standard
(Unity) desktop?
