The only hardware between the card and the monitor is the little dongle that converts DVI to VGA. These have always been connected to the card.
I was previously running okay with nouveau, and yes, had both displays at 1680x1050. Yes, I switch on twinview in nvidia-settings. I have now booted with only one display connected.
