Clues:

1. This was an "Erase disk and install Ubuntu" installation from a USB
stick, with "Install updates" and "Install this third-party software"
both checked.

2. While the problem was happening:
    - some applications in Main showed up in USC with icons
    - some applications in Main, such as Thunderbird and Pidgin, showed up 
without icons
    - the screen for anything in Main said  "Available from the 'main' source ( 
Use This Source)"
    - "Use This Source" triggered an apport dialog
    - apport refused to report a bug as described.

3. The problem went away completely as soon as Ivanka chose "Use This
Source" for an application in Universe. (So I guess "sudo apt-get
update" would have fixed it too.)
