Run this command (in a terminal):
apport-collect 712053
That should collect the required information, and open your browser. You
then log in and it automatically uploads/attaches the details to this
bug, I believe.
Also, can you get to Connection Information in Network Manager? If so,
what does it show next to "Driver:" ?
