Had a look at this again (although on a debian squeeze install) and
reading my fix description above I think that
1. "ldapsearch -LLL -Y EXTERNAL -H ldapi:/// -b cn=config dn" may not work
2. config need only contain the following:
dn: olcDatabase={0}config,cn=config
changetype: modify
add: olcRootDN
olcRootDN: cn=admin,cn=config
dn: olcDatabase={0}config,cn=config
changetype: modify
add: olcRootPW
olcRootPW: {MD5}Xr4ilOzQ4PCOq3aQ0qbuaQ==
I may be doing a 10.04 install in the next few weeks, if so I will confirm.
