I haven't done multiple upgrades like you describe. The "InstallationMedia: Ubuntu 9.10 "Karmic Koala" - Release amd64 (20091027)" you mention was an option in Lucid and must have begun with Karmic as it was the first version I installed cleanly. That option is now gone in Natty and now it says "To install from a CD-ROM or a DVD, insert the media into the drive". I remember that in previous Ubuntu releases you could add and remove sources there but that option has now been simplified.
