This occurs when you enable/disable plugins like ”Screenshot“, “Unity MT Grab handles” or “Title bar info”. As seen in the screenshot of comment #6 (I took it), elements in top panel (like the menus and the Ubuntu button) still work --you can press Alt+F and the “File” menu comes up, as usual. But the panel is blank until you reboot.
