I agree with the OP, this is quite a serious usability issue in the otherwise 
pretty
neat overlay scrollbars. In practice, the longer the document, the more 
important
scrolling, the less reachable the scrollbar.

A very simple solution - for which I can't really see any drawbacks - would be 
to 
just show the scrolling thumb every time the mouse reaches the right edge
of the screen _in every y position_ not only when in proximity of the orange
position indicator, which gets tiny as the document gets larger.
