Also seeing this problem on a Toshiba Satellite L515 with Ubuntu 11.04, insyde 
H20 BIOS (latest version)
Things to note:

1) It wasn't happening on 10.10, only after upgrading to 11.04
2) If I wait until the CPU heats up and then reboot, the fan starts working at 
full speed and continues to do so until shutdown, even with temperatures around 
31 C. I have tried the "wait 30 seconds inside GRUB after cold start before 
booting the OS", but it didn't work (guess it wasn't enough temp buildup for 
the fan to start)
