Ok, so I found a way to reproduce this reliably. It happens on the first 
hibernate attempt after a suspend/resume cycle.

1. Boot into Ubuntu.
2. Hibernate Ubuntu for the night
3. Wake Ubuntu up in the morning.
4. Suspend Ubuntu after using the system for a few minutes
5. Let the system sit in suspend for a few minutes.
6. Wake the system up.
7. Attempt to hibernate after a while.
8. Hibernate fails.

I'm not sure if the timing between these steps matters. I will do some 
more testing to see if these steps can be done in quick succession.
