It's easy to reproduce and has happened twice. Position a window so it's spanning across two screens, set unity to show the launcher when you mouseover the button on the top right and just grab the window by the part that would be covered by the unity bar if it was showing and you should end up stuck with a grabbing cursor unable to do anything and compiz won't respond to anything, even keyboard shortcuts.
