Update: I have determined that this bug is related to hardware. I was attempting to install from an IDE (or PATA) DVD drive to a SATA hard drive. I identified this issue with the installer for Ubuntu Server 10.04 - see bug https://bugs.launchpad.net/ubuntu/+source/linux/+bug/682159
The Ubuntu 11.04 alternate CD will not install from an IDE DVD drive to a SATA hard drive. The symptoms are identical to those described in bug 682159.
